Il termine kathoey (in thai: กะเทย) è usato in Thailandia (e anche in Cambogia e Laos) per indicare persone che possono essere donne transgender o uomini gay effeminati, spesso percepiti come appartenenti a un terzo genere distinto da uomo o donna[1][2][5].

Nonostante in inglese venga spesso tradotto come “ladyboy”, in Thailandia il termine non corrisponde esattamente al concetto di transgender: molti kathoey si identificano ancora come maschi con espressione femminile, mentre le donne transgender preferiscono termini come phuying (“donne”)[1][2].

Storicamente, kathoey indicava anche persone intersessuali e ha assunto nel tempo un significato più ampio, includendo uomini che mostrano tratti femminili e che possono o meno sottoporsi a trattamenti medici per assumere un aspetto più femminile[2]. Il termine può essere considerato anche leggermente dispregiativo in alcune forme, simile a “fairy” o “queen” in inglese[2].

In sintesi, kathoey è un concetto culturale thailandese che rappresenta una categoria di identità di genere e orientamento sessuale specifica del Sud-est asiatico, distinta dalle categorie occidentali di transgender o gay[1][2].
